Significant Event Policy

At Talbot Medical Centre, we are committed to providing safe, high-quality care for all our patients. We believe that learning from things that go wrong, or nearly go wrong, helps us to continually improve the services we provide.

What is a significant event?

A significant event is any unexpected incident that has, or could have, affected the safety or wellbeing of a patient. This includes incidents where no harm occurred, but where lessons can still be learned.

How we respond

When a significant event is identified, we will:

  • Investigate what happened in a fair and thorough way.
  • Review the circumstances to understand why it happened.
  • Identify any improvements that can be made.
  • Put changes in place to help prevent similar events happening again.
  • Share learning with the practice team where appropriate to improve patient safety.
